ENCLOSURES:

Please provide us with the following Document where applicable.

(1) Copy of your Birth Certificate

(2) Copy of your valid Passport

(3) Copy of your Health Insurance Policy, if any.

All this Documents must be fax to our Office Fax Number above.

APPLICATION FEES: This application fees is Three Hundred and Ten Euros (310 Euros) including your Visa Fees and Accommodations in the Netherlands, each applicants will be responsible for their Flight Ticket, which is one way Ticket to

the Netherlands.
